This case report aimed to evaluate the care needs of a patient with a diabetic foot ulcer within the framework of Kolcaba’s Comfort Theory and to present the nursing care process structured according to the theory.
A 50-year-old male with a 16-year history of type 2 diabetes mellitus was admitted to the clinic with burning, numbness, tingling, and pain in both feet and an ulcer on the right great toe. The patient had a previous history of left great toe amputation due to a diabetic foot ulcer. The ulcer on the right great toe was classified as Wagner Grade 1 by a specialist physician. The patient’s comfort needs were assessed according to the physical, psychospiritual, environmental, and sociocultural contexts of Kolcaba’s Comfort Theory and the comfort types of relief, ease, and transcendence. Individualized nursing interventions were planned and implemented according to the identified needs. The Visual Analog Scale pain score decreased from 6 at the beginning of care to 2 on the second day, and the patient reported no pain on the fifth day. The General Comfort Questionnaire score was 2.92 at the beginning of the care process and 3.76 at the end of care.
In this case, Kolcaba’s Comfort Theory provided a framework for systematically assessing the physical, psychospiritual, environmental, and sociocultural needs of a patient with a diabetic foot ulcer and for structuring nursing care accordingly. Although favorable changes in pain and comfort scores were observed during the care process, these findings from a single case should not be interpreted as evidence of a causal effect of theory-based nursing care.
